How to Enroll

Thank you for your interest in the R/K CAA Head Start Program . The application process is designed to be convenient and family-friendly. There are two simple steps that need to be completed to put your child on the road to Head Start participation.

1) Obtain and Complete a Head Start application

Head Start applications are available at any R/K CAA Head Start Program sites as well as any R/K CAA site. See the site listings for location nearest you. For your convenience the application packet is also available for download here. If you are unable to obtain the application and would like one sent to you please call: (262) 633-0082 to request one by mail.

2) Submit required documentation

Along with the completed application it is a requirement to include additional family information. Below is a list of documents that are required for student enrollment.

Proof of Birth

Child birth certificate, hospital birth records, official foot prints, or baptismal records are acceptable forms of documented proof of birth.

Immunization Record

Record of your child’s most recent vaccination history. Immunizations records can be obtained from your local pediatrician, Wisconsin Immunization Registry website or personal immunization log. Proof of Income

Per Head Start Performance Standards family income must be verified by the Head Start Program before determining that a child is eligible to participate. Acceptable forms of income verification include but are not limited to the following:

The preceding year's tax forms or W-2 statements.

Check Stubs (12 months)

Employer letter verifying annual income

Kinship care payments, Social Security and Supplement Security Income
