To get a referral, the following may be requested:

1. Picture ID (Driver's license, State ID, Human Services Department or Work ID)
2. Cards, Print outs, Medical cards, Insurance Cards for each person you need food for
3. Income verification (Check stub, statement letter)
4. Verification of Household size (Medical cards, Immunization Records, Birth Certificates)
5. Proof of Current Address (Utility or Telephone bills, rent receipt, etc)
