What to Bring to a WIC appointment:

  • Person (s) who have the WIC appointment
  • Proof of Identification for guardian and everyone being enrolled
  • Proof of current income
  • Proof of current address
  • Proof of pregnancy
When you make your appointment, WIC staff will review with you what to bring in for your appointment and give you examples of what is WIC-accepted in each of the above categories.
